Students: Employed or Distance Education Clinical Sign-up

This form should be completed by candidates who are completing clinical assignments (prior to the yearlong internship placement) either:

• in their own classroom as an employed teacher/TA (i.e., a residency teacher, graduate teacher of record, teaching assistant) 
• in a distance setting (i.e., in a district that is not considered a local district partner). Local district partners include Cabarrus, Catawba, Charlotte-Mecklenburg, Cleveland, Gaston, Iredell-Statesville, Kannapolis, Lincoln, Rowan-Salisbury, Stanly, and Union Counties.

If you are a distance education candidate (who lives more than a 50-mile radius from UNC Charlotte) and you need to secure a site for your clinical experiences (meaning you are not already employed in a school), you must first confirm that we have a partnership agreement with your desired school or district in order to complete your clinical experiences in that school or district. You can find a list of current partnership schools and districts on this webpage. Please note that partnership agreements can take up to 6 months to complete. This may delay the completion of your course requirements. You may NOT begin your clinical requirements unless your private/charter school or district is on the current partnership schools and districts spreadsheet.  Please email oscp-coed@charlotte.edu if you would like to complete your clinicals in a charter/private school or district not currently on the list. 

Prior to completion of this form, candidates should ensure the district/school is not listed in the Sonia online clinical portal. If your preferred site is already listed in the Sonia online clinical portal, then you should request the placement through the Sonia portal. 

Traditional students (those who are not employed teachers) seeking placements with local district partners (i.e., Cabarrus, Catawba, Charlotte-Mecklenburg, Cleveland, Gaston, Iredell-Statesville, Kannapolis, Lincoln, Rowan-Salisbury, Stanly, and Union Counties) must use the Sonia online clinical portal to sign up for their placement. This form should not be completed for local candidates whose choice of clinical placement is unavailable in the Sonia portal.
